Please keep in mind this is not my preferred method of contact since you cannot add photos to this form, like you can with the Google form. You will not be able to send example / reference photos until I reply to your email, which will slow the process down a little.

Description of Tattoo

Examples of appropriate responses: "I want my daughters name in cursive on my wrist" or "I want to start a sleeve with birds and flowers starting at my shoulder". ✦ DO NOT ✦ write something generic like, "I want to start my sleeve". EXPLAIN what you want the tattoo to consist of (roses, snakes, text, flowers, etc).

Estimated Size of Tattoo — MEASURED IN INCHES

Examples of appropriate responses: "3inches by 4inches" or "About 2inches long and 1inch high". DO NOT use words like "medium" or "very small". Please actually measure if you are able to. ✦ PLEASE NOTE ✦ The size you want your tattoo might not be possible and some changes may need to be made.

Desired Placement of Tattoo

Examples of appropriate responses: "My left inner wrist" or "Back of my right calf".

Are you taking any medications or have any conditions that may effect stopping bleeding or wound healing?

Examples of medications: blood thinners / anticoagulants, immunosuppressants, chemotherapy drugs, etc.

Times of the day / week that generally work best for you

If you are offered an appointment, please be prepared to pay a booking and / or a design fee. It will be nonrefundable.
